By Filling out the Questionnaire below, I agree neither The Baseball Company / The Baseball Academy - The Baseball Company Invaders / Canes Baseball, nor any employee or contractor, agent or partner of The Baseball Company Invaders / Canes or provider of facility or field for a tryout or practice will be liable for any damages, including without limitation, direct, incidental, consequential, indirect, or punitive damages, arising out of or relating to any use of The Baseball Company - The Miller School or agent partner facilities or equipment, baseball instruction or practice, training sessions, tryout facility, or any other services provided by The Baseball Company / The Baseball Academy / Canes Baseball or any employee, agent, or partner of The Baseball Company / The Baseball Academy or Canes Baseball is advised of the possibility of such damages. This limitation includes but is not limited to, personal injury, including death as well as damages to personal property.
